How to delete tag even I remove tag from each page

How to delete tag even I remove tag from each story but after story change name, the tag wouldn't remove.

For example
First time, I add Tag for "Willow love story". but later story change name to "Whispers Through the Willows".
in my tag, story name won't change. it still be "Willow love story", if i click it, the page go to "Whispers Through the Willows". I can't get back to page of willow love story and remove tag while the page of willow love story show as whispers through the willows"
